Background
Seminal vesicle tumors may present as retrovesical masses. Most of these tumors are silent and become symptomatic with compression of adjacent organs and nerves. Malignant progression is rare, nevertheless surgical excision may be required to resolve local symptoms. We present a robotic-assisted laparoscopic excision of a retrovesical mass with origin in the seminal vesicle.
Case Presentation
Male patient, 72 years old man, non-smoker, with past medical history of hypertension, medicated with lisinopril. Patient was referenced to our clinic for moderate LUTS (IPSS 13). PSA was normal and imaging documented a retrovesical nodular lesion with 5 cm of diameter adjacent to right seminal vesicle.
The patient was proposed for a robotic-assisted laparoscopic excision. A 12 mm port above the umbilicus and 2 ports were placed bilateral in the direction of the iliac crests. Access to the posterior aspect of the bladder was performed and complete dissection of the mass was accomplished. Anatomopathological report was compatible with schwannoma. Operative time was 50 min and estimated blood loss was 100 mL. The patient was discharged 2 days after the surgery. He had a complete resolution of his complaints.
Conclusion
Schwannoma is a benign tumor of the peripherical nerves sheath composed by schwann cells. It can cause compressive local symptoms. Robotic surgery is a safe and effective approach for the surgical management of this condition.
